Haven't started one of these in a while. I had this Tasma KN-3 film (exp. 1983) in my Nikkormat FTn. I shot and shot until the counter said 38. Now I never roll much over 27 frames, so I got suspicious and took it in the dark to check. Yep, the film had let go of the takeup spool, but for some reason I haven't figured out, the supply (rewind) knob was turning when I wound the film. Weird. Anyway, I restarted and shot the roll (25 exp). Then I went to develop in my old standbay Parodinal 1:25. I haven't used it in a while, but I was so confident that it was still good, that I just dipped the test strip at the same time I started the dev cycle in the tank. Well, that was a bad choice because my Parodinal was dead. I figured I could salvage something by throwing in some HC110 on top of the dead developer. I had no idea how long to let this concoction soak, so I gave it 10 minutes. That was too long. I got some serious base fog, but by and by, the negs were still more or less scannable using a dslr and a strong backlight. Here are a couple.
